Liability Insurance |

Students are required to have Student APN insurance prior to participating in clinicals. Proof of insurance must be presented to the Department Assistant before you will be allowed to proceed in the clinical setting. Some possible insurance agencies are: |

Immunizations |
| |
Before you proceed in clinical you must provide proof of immunization status. You are required to have proof of: |

MMR or positive titer |

Varicella or positive titer |

Hepatitis B or positive titer |
| • |
Negative PPD within the past year (or negative chest x-ray if unable to have PPD) |

Documentation of all of the above must be provided to the Department Assistant prior to beginning clinicals. |

Fingerprinting |

We need to have a copy of your official report from BCI regarding criminal background check (fingerprinting). Fingerprinting can be arranged at University Protective Services. You need to make the arrangements on an individual basis. |

Electronic FBI printing fee is $35. All inclusive.
To help you complete the fingerprint process quickly, please bring your university ID, any documentation requesting fingerprinting, and a drivers' license or state-issued ID.
