| Informational packets were developed to assess post-partum depression and provide mother/parent with information on newborn care. The packets include a family history questionnaire, Edinburg screening tool and information on child development and feeding schedule. A list of locations to ensure car seats are installed properly was also included. The team developed 2 color-coded packets to be distributed at the first 2-week and 2 month well childcare visit. Packets are organized for easy use. They include yellow/green & white forms to indicate which forms need to be return to provider after completion (yellow/green). An evaluation form was also included to assess the patient?s response to the process of completing forms in advance and the usefulness of the childcare information. The project was implemented mid January and providers are currently collecting results. Providers are also documenting the Edinburg screening results in Epic and intervening as indicated. |
